Incoming Students

The following steps must be completed prior to attending classes:

1. Download the Immunization History Form (required)

  • Print the form and complete the demographic (top) portion of the form.
  • Ask your physician or nurse to complete and sign the Immunization History form (physician’s office address and phone number must be included on the form). In lieu of completing the form, you may attach a copy of school immunization records, physician records or military records to the form. You may also receive required immunizations at McKinley Health Center’s Immunization and Travel Clinic for a fee.
  • Upload your documents to MyMcKinley

  • If you experience difficulty with the upload process, you may fax your documents to (217) 244-1278, mail or hand deliver to:

2. International Students ONLY: TB Screening Form (required)

  • Print and complete the TB Screening Form
  • Bring this completed form and a copy of your Immunization History Form with you to campus. Do not fax your TB Screening Form. 
  • You must provide a certified English translation if immunization records are not in English. If you do not have adequate documentation of your vaccinations, you may repeat the shots before coming to the United States or the shots can be provided at the McKinley Health Center for a fee.
 

3. If you are under the age of 18 at the start of your first semester:

 

NOTE: 

Within 2 weeks of receipt, you will receive an email notice to check your compliance status using MyMcKinley.

If you fail to submit your forms, you will not be able to add or drop classes in your current semester, and you will not be allowed to register for classes for subsequent semesters until you are in compliance.

4. Optional Disclosure of Private Mental Health Information​​ (not required)

In compliance with the Student Optional Disclosure of Private Mental Health Act, the university has established procedures for students to have the opportunity to authorize, in writing, the disclosure of certain private mental health information to a designated person.
